The letters go out, every plan is gone, and Monday starts with a fee-for-service schedule. That picture keeps more doctors stuck than anything else, because it turns the whole thing into a cliff.

In my book I wrote: “You could still accept insurance, but not be dependent on it. It has to do with how you set your prices.” Independence is about who sets the rules in your practice. Your patients can still have dental benefits. You can even keep a plan that pays close to your fees. What changes is that a payer’s fee schedule stops deciding your schedule, your time in the chair, and what you take home.

If you’re still deciding whether to leave at all, start with whether your practice is ready to drop PPOs. That page covers the numbers to check and the levels of insurance dependence. This page picks up after the decision. It’s the how.

Why So Many Dentists Are Asking Now

You’re far from the only one thinking about this. In ADA Health Policy Institute data from late 2025, more than one-third of dentists said they plan to drop some dental insurance networks. In an earlier ADA survey of dentists who had already left a network, 98.7% named reimbursement as a reason, and 57.3% named administrative burden.

That second number matters for your plan. If low reimbursement is the whole problem, the fix is mostly math. If the paperwork is wearing your team down too, then how you handle billing after the switch needs as much planning as the exit itself.

The Seven Steps, in Order

Each step depends on the one before it. Skip the contract review and the patient letters go out with the wrong date. Skip the team prep and your front desk is making it up the first time a patient calls to ask.

Step 1: Run the Numbers Plan by Plan

Build one table with a row for every plan you’re in. For each plan, fill in:

How many active patients are on it

What you actually collect per procedure, compared with your full fee

How many new patients found you through that plan last year

How much team time it eats in claims, follow-ups, and downgrades

Use collected fees, not listed fees. Chris Salierno, DDS, put it plainly when he wrote about dropping a PPO: “Just because a fee schedule lists a D2740 at $800 does not mean you can expect to collect that much.”

The new patient column surprises people. When Alexander Matheson, DMD, started asking every new patient how they heard about his practice, only 6% to 7% said they came through their insurance. Your number will be different. Ask the question for a few months before you guess.

Then hold each plan up against the daily production your practice needs to cover overhead and pay you properly. I call that your Magic Number. A plan that pays well below it isn’t bringing you patients. It’s renting out your chair time at a discount.

Step 2: Read Every Contract Before You Tell Anyone

This is the step people skip, and it’s where exits go wrong. Before a single patient hears about the change, pull every contract and check four things.

The notice period. Every contract sets its own. Delta Dental of Ohio, for example, asks for 30 days written notice to leave its EPO program. Salierno’s contract kept him in network for 60 days after he gave notice. Neither number is a rule. Your contract is the rule.

The effective date. That’s the day you’re actually out of network. It decides which booked patients need a letter and what their next visit will cost.

Affiliated carrier clauses. Some contracts let your network status be leased to other carriers, so you can be a participating provider for a plan you never signed with. The ADA’s guidance on dental insurance contract issues suggests asking for written notice and a chance to opt out. If you leave the main network and forget the leased ones, you haven’t really left.

Treatment in progress. Check what the contract says about patients who are partway through treatment when your participation ends.

If you’re an ADA member, the ADA offers a contract analysis service and a guide on terminating network agreements. Use them before you send anything.

Step 3: Pick the Order You’ll Drop Plans

There are two ways out. You can leave every plan at once, or you can leave one plan at a time and watch what happens after each one.

All at once fits when your numbers from Step 1 show that few new patients come from plans, and no single plan carries a big share of your active patients. One at a time fits when one plan is a large part of your patient base, or when you want to see how your patients react before you go further.

If you go one at a time, the order usually writes itself. The plan with the lowest collections and the fewest patients goes first. A plan that pays close to your full fee can stay as long as you like. Matheson dropped most of his plans but kept one that paid within 15% of his full fees. Salierno sized up his worst case before he moved: about 7% of his patients were on the plan he dropped.

I’m not going to give you a deadline here, because the right pace comes from your contracts and your numbers, not from a calendar. The order is what you control.

Step 4: Get the Team Ready Before Patients Hear Anything

Your team hears it first, and they hear why. A team that understands the decision will explain it with confidence. A team that’s surprised by it will apologize for it, and patients hear the apology.

Matheson’s example of what not to say is the one most front desks reach for: “Sorry, we don’t take your insurance anymore.” It sounds like the practice lost something. He recommends a positive, matter-of-fact explanation instead, and practicing it out loud. In his words: “Roleplay as if you were the patient and they need to answer your questions.”

Give the team answers to the questions they’ll get every day:

Can I still use my benefits here?

What will my next visit cost me?

Why are you doing this?

Will I still see the same doctor and hygienist?

These are the same conversations that decide whether a patient says yes to treatment, which I cover in why patients say no to dental treatment. A team that’s good at one gets good at the other.

Step 5: Tell Patients Clearly, and Early

Most of the work in this transition is quiet. The numbers, the contracts, and the team prep all happen before anyone outside the practice knows. Announcement day gets all the attention, but by then the outcome is mostly set.

Tell patients in person at their visits, before the effective date. For anyone booked after that date, send a written notice too. Salierno sent a polite letter to every patient on the plan who was scheduled past his in-network window. Matheson adds a point worth hearing: if patients trust the practice, the exact wording of the letter matters far less than the team worries it will.

Be specific about what changes and what doesn’t. What changes is the fee and how their claim gets handled. What stays the same is the doctor, the team, and the care. Patients whose plan has out-of-network benefits need a different explanation than patients whose plan pays nothing outside its network, so sort your list before you start the conversations.

Expect some patients to leave. Matheson was prepared to lose up to 30% of his PPO patients and lost well under that. That’s one practice’s account, not a forecast, but it’s a useful reminder that the fear is usually bigger than the loss.

Step 6: Set Up Billing for Life After the Contract

Decide ahead of time whether you’ll still file claims for patients, and who the plan pays, you or the patient. Patients who leave with a clear answer to “how do I get reimbursed?” are far more likely to stay.

Out of network, the plan sets its own terms. According to the ADA’s guidance on out-of-network providers, plans may reduce allowable fees and apply higher deductibles and lower maximums, and the only reliable way to know what a plan will pay is to contact the carrier. The ADA also recommends giving patients a good faith estimate before treatment, which can reflect your full fee.

For the plans you keep, know your state’s rules. 44 states have noncovered services laws, which limit whether a plan can set your fee for services it doesn’t cover. The six without one are South Carolina, Delaware, New York, Michigan, Massachusetts, and Hawaii. “Not covered” and “covered but not paid,” such as a service over the annual maximum, can be treated differently, so read the wording for your state.

Step 7: Fill the Gap Before It Opens

Insurance directories sent you some new patients. When you leave, something has to replace them, and it needs to be running before the letters go out. Salierno’s advice is to start marketing campaigns before you cut the cord.

The strongest replacement is the practice your current patients already know. Referrals and reviews come from patients who had an experience worth talking about. After leaving his PPOs, Matheson leaned on Google reviews and made a habit of asking for referrals. Start asking now, while the patients who like you are still coming in for their cleanings.

How to Measure the Transition

Track a short list of numbers every month, starting the month before your first notice goes out:

Payer mix of your active patients

Average collected per visit

How many patients from dropped plans keep their next hygiene visit

Where new patients come from, using the same intake question from Step 1

How full the schedule is

Hours the team spends on claims

Review them together, in the same monthly review you’d use to find profit leaks. If collected per visit is rising and hygiene retention is holding, the plan is working, even if the patient count dips for a while.

Where Your Rules Stop

The contract controls the exit. Your notice period, your effective date, and how leased networks are handled all come from the contract you signed. Read it, or have it reviewed, before you set a date.

Plan type changes the rules. Fully insured plans are usually regulated by the state. Self-funded employer plans are usually governed by federal ERISA rules and may not follow state insurance laws, including noncovered services laws.

State laws aren’t uniform. Noncovered services laws exist in 44 states and differ in what counts as “covered.” Don’t assume your state works like a colleague’s.

Some questions belong with an attorney. That includes obligations to patients in the middle of treatment, any plan to coordinate a network exit with other dentists, how you’re allowed to advertise the change, and Medicaid or CHIP participation.

This section is general information, not legal advice. Check your state dental board rules, your payer contracts, and a healthcare attorney before you send a termination notice.

Where to Start This Week

Build the table from Step 1. One row per plan, with four columns: active patients, collected fee as a share of your full fee, new patients from that plan last year, and the notice terms from the contract. You’re done when you can name the first plan you’d drop and the date its notice window would open.

Related FAQs

How much notice do I have to give a dental insurance plan before I leave?

It depends on your contract, and there’s no single standard. One Ohio carrier asks for 30 days written notice for its EPO program, and other contracts run longer. Read the termination clause in each contract before you set a date.

Can I still file insurance claims for patients after I leave a network?

Yes, you can file claims for patients as an out-of-network provider. The plan may pay less, apply a higher deductible, or lower the annual maximum, so call the carrier to confirm benefits and give the patient a good faith estimate before treatment.

Can I charge my full fee for a service the plan doesn’t cover?

In most states, yes. 44 states have noncovered services laws that limit plans from setting fees for services they don’t cover, but the details vary by state, and self-funded employer plans may not follow state law. Check your state’s wording before relying on it.

Do I have to drop every plan to be insurance independent?

No. Insurance independence means a payer’s fee schedule no longer runs your practice. Many practices keep one plan that pays close to their full fee and leave the rest.

If you want help working out which plan goes first in your practice, book a Lifestyle Practice Blueprint call and we’ll look at your plan-by-plan numbers together.
